home52 is a new name around town, but the people behind home52 have been working to support the needs of older adults, people with disabilities and caregivers in our community for more than 50 years. You know and trust these people as Council on Aging (COA). Leveraging more than 50 years of experience, we’re experts at helping individuals, families and caregivers manage complex medical and long-term care needs.
Older adults and people with disabilities often need specialized transportation services, including special vehicles, assistive devices, through-the-door, and on-demand service. When local transportation networks can’t meet these unique needs, whole segments of our population experience a decline in health and quality of life.
home52 Transportation manages multiple transportation providers through a centralized transportation coordination system, providing specialized, on-demand and advance-scheduled trips for older adults and people with disabilities.
Additionally, for providers such as hospitals, health systems, dialysis clinics and federally qualified health centers, home52 Transportation can act as a centralized scheduling service, optimizing routes, schedules and vehicles to best meet the unique needs of their riders.
home52 Transportation provides a different transportation experience than traditional commercial, public transit and rideshare services. home52 Transportation was designed to support the unique transportation needs of older adults and adults with disabilities. Our service includes:
“The individuals who ride with us need some level of extra support to travel independently in the community. They may need hands-on support. They may need someone to meet them at their door, assist them safely in and out of the vehicle, and then assist them into their appointment location. Our drivers also manage the rider’s mobility aids, such as walkers and wheelchairs. We use a person-centered approach when providing transportation support by offering the highest level of support to every rider and allowing our riders to choose the level of support they want” – home52 Transportation Manager.
We currently provide services in Hamilton County, Ohio. We are planning to expand our service area to Clermont County, Ohio in 2025.
